Encompass Health offers inpatient rehabilitation services where patients and their designated caregivers can utilize the MyEncompass Health Caregiver application to stay updated on the patient’s rehabilitation journey. This secure application displays real-time progress towards goals and an overview of the patient's care plan, regularly updated by our dedicated care teams to align with individualized plans.

• TRACK
  • Monitor the patient’s progress towards established goals, including mobility, self-care, cognition, behavior, and daily living skills.
  • Access information such as anticipated discharge date and preparations for post-hospital care.
  • Receive guidance on discharge diagnosis, post-care instructions from nursing and therapy, follow-up appointments, medication details, and other essential home care instructions.
• SHARE
  • Invite others to track the patient’s rehabilitation journey.
  • Enable the patient's legal representative to share information by granting access on their behalf.

Access to the application is extended up to 14 days post-discharge for both the patient and authorized users.
